I have doubts regarding late effects, i have coded , but wanted to check if my answers are correct.
  1. Hemiplegia due to previous cerebrovascular accident.
  2. Malunion of fracture right femur.
  3. Scoliosis due to old infantile paralysis.
  4. Keloid secondary to injury 9 months ago.
  5. Mental retardation due to previous viral encephaliti
  6. Traumatic arthritis of the right shoulder due to old fracture of right humerus
  7. Paralysis of left leg due to old poliomyelitis
  8. Scoliosis due to poliomyelitis at age 12
  9. Traumatic arthritis of the right shoulder due to old fracture of right humerus
  10. Paralysis of left leg due to old poliomyelitis
  11. Patient with residual aphasia due to subdural hemorrhage two years ago who is admitted because of acute cerebral thrombosis would have the following codes
  12. Cerebrovascular accident two years ago with residual hemiplegia of the dominant side
  13. Contracture of hip following partial hip replacement one year ago
  14. Traumatic arthritis right ankle following fracture right ankle
  15. Cicatricial contracture of left hand due to burn
  16. Brain damage following cerebral abscess seven months ago
  17. Flaccid hemiplegia due to old cerebrovascular accident
  18. Bilateral neural deafness resulting from childhood measles 10 years ago
  19. Mononeuritis median nerve, resulting from previous crush injury to right arm
  20. Posttraumatic painful arthritis left hand
  21. Residuals of previous severe burn left wrist
  22. Locked in state (paralytic syndrome) due to old cerebrovascular accident
Should we first go to late effects in the alphabetical list or code first for todays treatment.
Everytime i think that i have got it right, but still i am not coding it correctly

I get you started with one ;)

  1. Malunion of fracture right femur.
The malunion is the reason they walked through the door so look that up and code as usual. To tell the story that the malunion is due to an old fracture you go to the index under the entry "late" the the sub-term "fracture" lower extremity and code that as a secondary code.

Also when you want others to check your work it's best to show your work - like give us your answers and we can quickly look them up to let you know if they are correct.

I will take a look at this on Thursday, October 24, 2013 and get back to you of my findings. I do notice that you do not have #2 Malunion of facture right femur coded.

is it necessary that all late effect codes should have 2 codes, questions 11, 12, 16, 20, i am not able to think of a code more than this, above all thanks for your reply and help.

No if you can tell the story of effect and cause in one code then that is fine.

The ones that are in red are changes that I have made. 438.20 does not need a second code. 737.43 has certain associated diseases that must be coded first and infantile paralysis is not included in them. Numbers 1 and 6 are identical. 326 is for the late effects of codes 320-325 excluding 320.7, 321.0-321.8, 323.01-323.42, and 323.6-323.7. 326 also has two additional codes to identify the condition that must be coded and 348.9 is not one of them.

Tip: Always read notes and instructions in the coding manuals.
